About the Department
The Department of Pharmacy Practice at the College of Pharmaceutical Sciences was founded in 2013 with a multifaceted approach contributing to patient care and education. Its primary goal is to empower students to excel in comprehensive medication management, enabling them to provide exceptional patient care. Alongside this, the department also focuses on nurturing research and leadership skills, as well as promoting a culture of lifelong learning among its students. The department has established the Drug Information and Patient Counselling centre at most renowned hospitals i.e. Sagar Hospitals, Bengaluru and at Dr. Chandramma Dayananda Sagar Institution of Medical Education and Research (CDSIMER) tertiary care hospital. The department is comprised of a team of dedicated faculty and bring a wealth of expertise and experience, with a strong focus on both clinical knowledge and pharmaceutical care.
The department provides Drug Information services, Medication Therapy management, Patient Counseling, Medication Reconciliation, Pharmacovigilance, Community Outreach program, Health Screening programs, Clinical research, and Continuing Pharmacy education programs.

- Prescription Audit: The center evaluates the appropriateness and safety of prescriptions and ensures adherence to standard guidelines and recommends improvements to optimize patient outcomes.
- Patient Counselling: The center provides personalized counseling to patients, empowering them with information about their medications, including dosage, administration, potential side effects, and precautions.
- Medication History Interview: The center collects comprehensive data regarding a patient's medication usage, including prescription drugs, over-the-counter products, and supplements. This information facilitates better clinical decision-making and reduces the risk of adverse drug interactions.
- Adverse Drug Reaction: The center monitors and investigates adverse drug reactions (ADRs) in collaboration with healthcare providers and analyzes data to enhance patient safety, identify potential risks, and improve medication-related outcomes.
- Cost Minimization Analysis: The center evaluates the cost-effectiveness of various treatment options, comparing their clinical benefits and financial implications thus helping in optimizing healthcare resources and ensuring the affordability of therapies.
- Treatment Chart Review: The center assesses the appropriateness and effectiveness of medication regimens and identifies opportunities for optimizing therapy and minimizing potential risks.
- Drug Information Query: The center serves as a reliable resource for healthcare professionals and patients, providing evidence-based information on drugs, including indications, dosage, adverse effects, drug interactions, and contraindications.
- Pharmacist Intervention: The center actively engages in clinical settings, collaborating with healthcare teams to provide pharmacist interventions to adjust medication regimens, resolving drug-related problems, and promoting rational drug use.
- Medication Incident Report: The center analyzes medication incident reports to identify system weaknesses, implement corrective measures, and improve patient safety practices.
- Antibiotic Audit: The center conducts audits of antibiotic usage to promote appropriate prescribing practices, reduce antimicrobial resistance, and optimize patient outcomes.
- Materiovigilance: The center monitor and evaluates the safety of medical devices, focusing on adverse events, device-related complications, and patient outcomes.
- Medical Reconciliation: The center emphasizes the importance of accurate medication reconciliation during transitions of care and strives to ensure seamless continuity of therapy and prevent medication-related errors.
